DA Form 7888 OPAT Scorecard: Download & Standards – DA Form 7888 is the official Occupational Physical Assessment Test (OPAT) Scorecard used by the U.S. Army. It officially records an individual’s performance on the four-event OPAT, which evaluates physical readiness for specific Military Occupational Specialties (MOS).
The current version of the form was issued in August 2021 and is authorized under ATP 7-22.01. It helps ensure Soldiers and recruits meet the physical demands of their assigned career field before training or reclassification.

Purpose of the OPAT and DA Form 7888
The OPAT matches Soldiers to jobs based on physical requirements, reducing injury risk and improving long-term readiness. The test is gender-neutral and age-neutral, measuring upper-body power, lower-body power, strength, and aerobic endurance.
DA Form 7888 serves as the official document to record test results. Recruiters, OICs, or NCOICs administer and document the scores, which are later entered into the Army’s official system.
The Four Events on the OPAT Scorecard
1. Standing Long Jump
Measures lower-body explosive power. Performed in centimeters (best of three trials).
2. Seated Power Throw
Assesses upper-body power using a 4.4 lb medicine ball thrown from a seated position (best of three trials).
3. Strength Deadlift
Evaluates total-body strength using a hex bar. Soldiers must complete three repetitions at the highest successful weight.
4. Interval Aerobic Run (IAR)
Tests cardiorespiratory endurance with a 20-meter shuttle run. Results are recorded by level and shuttle number.
OPAT Physical Demand Categories
Based on performance, individuals are placed into one of three categories:
- Heavy (Black) – Most physically demanding MOS
- Significant (Gray) – Moderately demanding jobs
- Moderate (Gold) – Least physically demanding roles
Each MOS has a required minimum category. You must meet the standard in all four events.
Current OPAT Scoring Standards (2026)
Heavy (Black)
- Standing Long Jump: 160 cm
- Seated Power Throw: 450 cm
- Strength Deadlift: 160 lbs
- Interval Aerobic Run: Level 6-2 (43 shuttles)
Significant (Gray)
- Standing Long Jump: 140 cm
- Seated Power Throw: 400 cm
- Strength Deadlift: 140 lbs
- Interval Aerobic Run: Level 5-8 (40 shuttles)
Moderate (Gold)
- Standing Long Jump: 120 cm
- Seated Power Throw: 350 cm
- Strength Deadlift: 120 lbs
- Interval Aerobic Run: Level 5-4 (36 shuttles)
Who Must Take the OPAT?
The OPAT is required for:
- All new Army recruits (Active Duty, Reserve, and National Guard)
- Soldiers requesting MOS reclassification
- Select ROTC cadets
How to Prepare for the OPAT?
Effective preparation includes:
- Compound lifts (especially deadlifts)
- Plyometrics and box jumps
- Medicine ball power throws
- Shuttle run / beep test training
How to Properly Fill Out DA Form 7888?
Steps include completing personal information, recording raw scores, circling the best attempts, and obtaining the grader’s signature. The form must be retained for official records.
